Memantine may be helpful for people with moderate-to-severe autism disorders.

The glutamate pathway may be out of balance in brains of people with autism. Memantine is a medication that reduces certain brain activities by blocking glutamate receptors in the brain. Two clinical trials of memantine in people with autism had different results. One trial reported that memantine was helpful for improving language and social interactions in people with pervasive developmental disorder-not otherwise specified (PDD-NOS). Another trial reported improvements in memory, irritability, and other behaviors for people with PDD. However, language was not affected in the second trial. Neither of these studies included a placebo control. Better studies are needed to find out if memantine may be helpful for people with autism.

This article gives an overview of the research on the biological causes of autism and potential complementary and alternative (CAM) therapies for autism.

The authors describe the symptoms of autism, and then briefly discuss the differences between the brains of people with and without autism. According to the authors, some of these differences suggest that the usual brain development seen in individuals without autism is somehow short-circuited in those with autism. This could be due to many factors, including differences in metabolism, digestion, and the production of neurotransmitters. Next, several medicines used to treat autism are described, many of which affect neurotransmitters or their receptors. CAM treatments are also described, including supplements, immune treatments, and oxytocin infusion.


Neuroscience, by Pierce, RC, Rowlett JK, Bardo MT, and Rebec GV, published in 1991, summarized Oct 21, 2006

This article demonstrates that rats treated with vitamin C (ascorbate) had similar behavior to those treated with the tranquilizer haloperidol, but that vitamin C and the tranquilizer acted through two different mechanisms.

It is known that in animals, vitamin C has similar effects as the tranquilizer haloperidol, so some researchers think that vitamin C and haloperidol act through similar mechanisms. In this study, the authors test this idea. First they show that rats exposed to a dopamine agonist (a dopamine-like compound) will respond with more sniffing behavior if they have been treated with low-dose vitamin C or haloperidol as compared to rats treated with placebo. Then, they examine the brains of these rats to see if the treatments caused changes in dopamine receptors. They found that rats treated with haloperidol alone had more dopamine receptors compared to rats treated with placebo, but that rats treated with vitamin C alone did not. The authors conclude that even though vitamin C and haloperidol result in similar behavioral effects, they do not affect the dopamine system in the same way.

One Person's Yummy Food Is Another Person's Yucky Food

Vegetables and protein (fish, poultry, meats, and beans) are the most common food aversions. Some children even reject fruits. The diet becomes extremely restricted to bland, white foods, including sweets, breads, pasta, crackers, pretzels, chips, and macaroni and cheese. These foods are glycemic and raise blood glucose, quickly increasing the demand for insulin production. Zinc is part of the insulin molecule and is depleted, resulting in abnormal taste and taste perception. What develops is an aversion to the flavors in natural foods and increased cravings for highly sweetened foods and those foods containing MSG. (MSG affects the brain's perception of flavor.) The diet becomes more narrow, and nutritional status declines, resulting in more limited food choices. The white diet and vegetable aversion is common among children in Western cultures due to the high exposure to processed and sweet foods. For children with sensory and developmental issues, the aversions are much more pervasive and serious.

There are three potential contributors that lead to the limited appetite, cravings, and food aversions:

  1. The formation of opiate-like peptides from gluten and milk products, resulting in cravings for the foods that are the sources.
  2. Zinc deficiency or deficiencies in zinc metabolism and function. Zinc is responsible for sensory development and function, including taste and taste perception.
  3. The presence of toxic metals that can interfere with sensory development and function.

The negative effect of this combination can result in faulty messages from the sensory receptors to the brain and dysfunctional interpretation of those messages by the brain. Perception is the "truth" for that person. This is why begging, bribing, and punishing do not and will not work.

The solution is multifaceted. Correction of nutritional deficiencies and elimination of toxic metals are mandatory and involve appropriate therapeutic supplementation based on findings. Foods that cause reactions and/or opiate-like peptides need to be eliminated as healthy, safe foods are introduced and accepted.

How to Go from Yucky to Yummy-The Trojan Horse Technique

For those with texture issues, it is important to adapt the diet to the child's oral and food developmental stage. If textures are a sensory issue, no matter how tasty the food, it will not be consumed. By providing the food in a sensory-pleasing form, the child benefits nutritionally and begins to find mealtime more pleasant and rewarding. Purees are generally helpful. They are better tolerated and can open the door for getting more types of foods into the diet. Many family dishes, including soups, casseroles, or the meat and vegetable main dish, can also be served pureed for the child who has sensory texture issues. In this way, the whole family is enjoying the same meal.

Many of the recipes in this book have been selected to expand nutritional intake, especially using the Trojan Horse Technique-hiding a small amount of the new food (especially vegetables and proteins) within a very well tolerated and acceptable food. Each child differs and, therefore, it is important to identify what foods will work as "carriers" to get the new foods in.

Purees can be made from cooked fresh or frozen vegetables and/or purchased baby foods. If your child is offended by being served baby food, simply keep it well hidden. Create interesting new names for the foods and see that others in the family join in consuming them. The secret to success in introducing these new foods is to combine a small amount with the food the child already likes. For many children, this is the only way new foods can be introduced.

Start with 1 tablespoon (15 g) or less-and then increase when tolerated. Hide the cooked vegetable purees anywhere you can, selecting colors that are not obvious when added to the carrier food. The carrier food needs to be one that the child enjoys. It may even be a food that is being slowly eliminated. Include pureed fruits to improve the taste. Here are some examples of places to hide foods (and even supplements):

  • Spaghetti Sauce. Blend the pureed vegetables thoroughly with at least three times as much spaghetti sauce; then hand-mix the new blend in with the rest of the sauce. Carrots, beets, sweet potatoes, turnips, squash, green beans, and peas are easy to hide in spaghetti sauce. Watch the amount of green if it is a food color that your child rejects.
  • Muffins, Cakes, and Brownies. Well·pureed foods are easy to hide in these batters, including pureed chicken and turkey. A chicken/vegetable/fruit muffin becomes a healthy meal!
  • Pancakes. Not only can pureed vegetables and fruits hide well in the batter, but they are also a good hiding place for supplements such as protein powders, calcium, magnesium, and zinc.
  • Peanut Butter. If a child likes peanut butter, it is an excellent medium for adding small amounts of protein and nutritional supplements.
  • Meatballs. If these are well liked, especially with spaghetti sauce, the job becomes a whole lot easier. Well-pureed vegetables and fruits are an excellent thickener/filler for meatballs. Make many and freeze them, and then bring them out for snacks.
  • Juices, especially those with a strong flavor, such as pineapple juice, grape juice, nectars, apple cider, and orange juice.
  • Smoothies, Fruit Purées, and Applesauce. These offer an unlimited opportunity for expanding nutrition and an excellent way to hide supplements. Protein powders can be included to expand protein intake, especially for those with texture issues who avoid meat, beans, and other sources of protein. Always start with the fruit your child favors and then expand.
  • Let chocolate be your friend. There are sources of GFCF chocolate chips, sauces, powders, and so forth. Check the product search section of the GFCF Diet site (www.gfcfdiet.com/directory.htm).
  • Add unflavored gelatin to cooked purees to lend more substance and improve digestion.

Essential Fatty Acids

I could go on and on about essential fatty acids (EFAs). They are important. They are found in breast milk. Only recently are they being put in formula and only in the more expensive lipil formula. They are similar to the cod liver oil that our parents may have given us. There are plant sources of EFAs (i.e. flax) and animal sources (i.e. fish). Animal sources appear to be better absorbed and more effective than plant sources. There is a good and well thought out article on EFA's that can be found at PWS Playroom (http://www.pwsplayroom.com/efa.htm ).

These unsaturated fatty acids are easily used by your body to form the brain and the lipid layer around cells. Saturated fats (like butter) compete with unsaturated fats. There is some talk that ingestion of EFAs contributes to brain formation and intelligence. Have you heard about breast fed babies being smarter? If it is true, it is likely due to EFAs. Have you heard of fish being brain food? If this is true it is likely due to EFA's.

Check out the Cherab Foundation on EFAs (http://www.cherab.org/information/dietaryeffects/efabasics.html ). There is a lot of anecdotal evidence about fatty acids and language development. I found this story persuasive.

Finally, there is the role of these EFA's in metabolism. I haven't seen much discussion about this. I would welcome anyone's opinion as this is a stretch for me. But, there are a group of receptors called PPAR. They bind fatty acids and they are involved in numerous diseases including diabetes. Saturated fatty acids appear to bind them and initiate an inflammatory response that can have many bad downstream effects, such as heart disease. Unsaturated fatty acids compete for these receptors and have an anti-inflammatory effect. I am mulling this over...

I have now switched to the Ultimate Omega formula. I am not convinced that we need more Omega-6's (present in the ProEFA blend) and would rather just supplement with the Omega-3's.

Working them into your Diet

There is an interesting book called the Omega Diet that talks about oils. In my opinion, the book is a bit extreme, but makes really good points. I think that a diet high in flax oils and fish oils is good for most everyone.

I think that if you get into the groove it won't be so hard to incorporate. Maybe it will "work" and maybe it won't but probably you will all be healthier. There are many places to work in flax oil. There is a type of yummy bread made by Natural Ovens that has high levels of flax oils. They also make great (but expensive) snack bars. Plus, there are great frozen waffles with flax oil.

Fish can be eaten for fish oil and fish oil is pretty easy to take in capsule form. My whole family takes it. We call it "smart medicine."

The Omega Diet also mentions walnut oil and canola oil. I make my own salad dressings (oil and vinegar and spice) and switched from olive oil to walnut oil. It was pretty easy and tasty. I don't really bake, but keep thinking that bran muffins or banana nut bread made with walnut oil would probably be pretty tasty.

I am also a big honey person. When my kids want something sweet, I give them a teaspoonful of honey. They like it. Local honey is best if you can get it.

Also, this year we made the switch from regular potatoes to sweet potatoes. I am not sure what your guy would say about sweet potatoes, but they suit us well. It seems that you could do most anything with them that you could with real potatoes. Plus, they have the added benefit that you can add walnuts (and walnut oil??) and cinnamon and honey to them and call them dessert. :)

From superior adaptation and function to brain dysfunction--the neglect of epigenetic factors.

With optimal pregnancy conditions (natural, enriched diet which includes fish) African (Digo) infants are 3-4 weeks ahead of European/American infants in sensorimotor terms at birth, and during the first year. Infants of semi-aquatic sea-gypsies swim before they walk, and have superior visual acuity compared with us. With adverse pregnancy behaviour (fear of fat, a trend to dieting), neglecting the need for brain fat to secure normal brain development and function, we run a risk of dysfunction--death. Sudden Infant Death Syndrome victims have depressed birth weight, lower levels of marine fat in brainstem than controls, and >80 suffer multiple hypoxic episodes prior to death. Depressed birth weight (more than 10% below mean) is seen in learning and behaviour disorders, and a trend towards weights of less than 3kg is increasing, which supports a rise in antenatal sub optimality. Given marine fat deficiency in pregnancy and infancy, neurons starved for fuel could delay myelination and maturation in the latest developed Frontal Lobes. The phylogenetic oldest Lateral Frontal Lobe System (feed-back mechanism etc.) derived from olfactory bulb-amygdala, which crosses in Anterior Commisure is probably spared, while the Medial Frontal Lobe System derived from Hippocampus-Cingulum and crosses in Corpus Callosum (delayed response task) is most likely affected. The rise in infantile autism (intact vision and hearing) with deficit in delayed response task only, could suggest a deficit in the Medial Frontal Lobe System. The human species is unique; 70% of total energy to the foetus goes to development of the brain, which mainly consists of marine fat. It undergoes pervasive regressive events, before birth, in infancy and at puberty. Minimal retraction of neuronal arborisation is advantageous. Attributable to adverse pregnancy childrearing practice, excessive retraction is likely prenatally and in infancy. Pubertal age affects the fundamental property of nervous tissue, excitability: excessive excitatory drive is seen in early, and a deficiency in late puberty. It is postulated that with adequate marine fat, there is probably no risk of psychopathology at the extremes, whereas a deficiency could lead to paroxysmal (subcortical) dysfunction in early puberty, and breakdown of cortical circuitry and cognitive dysfunctions in late puberty. The post-pubertal psychoses, schizophrenia and manic-depressive psychosis at the extremes of the pubertal age continuum, with contrasting excitability and biological treatment, are probably the result of continuous dietary deficiency, which has inactivated the expression of genes for myelin development and oligodendrocyte-related genes in their production of myelin. The beneficial effect of marine fat in both disorders, in other CNS disorders as well as in developmental dyslexia (DD) and ADHD among others, supports our usual diet is persistently deficient. We have neglected the similarity of our great brain to other mammals, and our marine heritage. Given the amount of marine fat needed to secure normal brain development and function is not known, nor the present dietary level, it seems unduly conjectural to postulate that a dietary deficiency in marine fat is causing brain dysfunction and death. However, all observations point in the same direction: our diet focusing on protein mainly, is deficient, the deficiency is most pronounced in maternal nutrition and in infancy.
